Eligibility includes a certain body mass index (BMI) and specific weight-related comorbidities (co-occurring health conditions). Starting in 2027, Foundayo and certain similar drugs are expected to be available under a program called the Better Approaches to Lifestyle and Nutrition for Comprehensive hEalth (BALANCE) Model. 

Does Medicaid cover Foundayo?

Is there a generic for Foundayo? 

There is no generic version of Foundayo. Orforglipron is the active ingredient in Foundayo, which is different than semaglutide and tirzepatide - the active ingredients in other GLP-1 agonists, like semaglutide (Ozempic, Wegovy), and GIP/GLP-1s like tirzepatide (Zepbound, and Mounjaro).

Is there a cheaper alternative to Foundayo?

Generic weight loss pills like phentermine and phendimetrazine are cheaper than Foundayo; however, these medications are typically intended for short-term use only.

Liraglutide and exenatide are the only other generic GLP-1s, which are cheaper than Foundayo, but liraglutide is a daily injection, and exenatide is a twice-daily injection rather than a daily pill.

The only other oral GLP-1s available are the Wegovy pill and Rybelsus, and Rybelsus is only used off-label for weight loss. Injectable GLP-1s and GIP/GLP-1s, like Ozempic, Wegovy, Zepbound, and Mounjaro, are more expensive than Foundayo.

What is Foundayo? 

Foundayo is used with diet and exercise for weight loss and long-term weight maintenance in adults with obesity. It can also be used in adults who are overweight and have at least one weight-related health condition, such as high cholesterol or high blood pressure. 

Like other GLP-1s, Foundayo works by slowing stomach emptying, which helps regulate appetite and food intake and promotes weight loss.

Foundayo is a tablet that is taken once daily, with or without food.

Foundayo is a unique GLP-1 as it is a non-peptide small molecule whereas other GLP-1s are peptide-based. Peptide-based drugs are more easily destroyed by stomach acid than non-peptide small molecules, which is why Foundayo doesn’t require taking it on an empty stomach. Foundayo is more effective than oral semaglutide in reducing A1C and weight, according to a 52-week study by the Foundayo manufacturer, Eli Lilly.
