Contrave (naltrexone/bupropion) is a brand-name drug used for weight loss. The FDA has not approved a generic version of Contrave, but one could come on the market after 2030 based on patent expiration data. Contrave is a combination of naltrexone and bupropion. Naltrexone is an opioid antagonist that is used on its own to treat drug abuse and alcohol addiction. Bupropion (Wellbutrin is a popular brand name) is an antidepressant medication that's prescribed on its own to patients with major depressive disorder. Bupropion may also be prescribed for smoking cessation or seasonal affective disorder. However, as a combination drug, Contrave affects different parts of the brain to suppress appetite and balance natural chemicals called neurotransmitters.
Contrave is prescribed for chronic weight management in adults with obesity or overweight with at least one weight-related condition such as high blood pressure or high cholesterol. Losing weight decreases your risk of certain conditions such as high blood pressure, heart problems, and Type 2 diabetes. For best results, you should pair this weight-loss medication with a low-calorie diet and increased physical activity, under the supervision of your healthcare provider.
Before you take Contrave, tell your healthcare provider about any medical conditions you have, including if you are pregnant or breastfeeding. Contrave is generally not used during pregnancy or breastfeeding.
Also, tell your provider about all of the medicines you take, including OTC and prescription medications, vitamins, and supplements. There are many possible drug interactions with Contrave, so your healthcare provider will check for drug interactions. You should minimize or avoid alcohol while taking Contrave.
If you take a urine drug test, tell the lab staff member that you are taking this medication, as it can cause false positive results for amphetamines.
Taking Contrave can increase the risk of opioid overdose, especially if you use opioid medicines. Let your family and close friends know about this risk. You can also ask your pharmacist for naloxone, which is available over the counter and can be used in the event of an opioid overdose. Seek emergency medical help if you experience trouble breathing, extreme drowsiness with slow breathing, faintness, dizziness, confusion, or other unusual symptoms. Also, avoid using any opioids for 7 to 10 days before starting Contrave to prevent severe withdrawal symptoms. Inform your healthcare provider about your use of Contrave before any medical procedures or surgeries.
Contrave is not safe for everyone. Do not take Contrave if you:
Have uncontrolled high blood pressure
Are pregnant or breastfeeding
Have or have had seizures
Take other medications that contain bupropion, such as Wellbutrin, Wellbutrin SR, or Wellbutrin XL
Have or have had an eating disorder like anorexia or bulimia
Are dependent on opioid pain medications or use medicines to stop taking opioids, such as methadone or buprenorphine, or are in opiate withdrawal
Drink a lot of alcohol and suddenly stop drinking
Abruptly stop taking sedatives, benzodiazepines (anxiety medications), or anti-seizure medicines
Are taking monoamine oxidase inhibitors (MAOIs). Do not start Contrave until at least 14 days after stopping an MAOI
Are allergic to any of the ingredients in Contrave
Contrave extended-release tablets contain 8 mg of naltrexone and 90 mg of bupropion. Your healthcare provider will guide you on how to take Contrave. If you do not lose a certain amount of weight after 16 weeks, your provider may advise stopping the medication. Avoid taking Contrave with high-fat meals, as this can increase your risk of seizures. Swallow the tablet whole. Do not chew, crush, or split the tablet.
The most common side effects include stomach problems like nausea, vomiting, and diarrhea or constipation. Other common side effects may include dizziness, dry mouth, headache, and trouble sleeping.
Contrave has a black box warning, the FDA's most serious warning. This warning states that antidepressants, including Contrave, can increase the risk of suicidal thoughts and behaviors, particularly in children, adolescents, and young adults. Anyone taking Contrave should be closely monitored for any changes in mood, behavior, or unusual thoughts, including suicidal thoughts or actions.
Serious side effects may occur. If you have symptoms of an allergic reaction, you should get emergency medical help immediately. Symptoms may include hives, shortness of breath, or swelling of the face, lips, tongue, or throat. Other serious side effects may include, but are not limited to, seizures, high blood pressure, liver problems, and angle-closure glaucoma. Talk to your healthcare provider about potential side effects and what symptoms to watch for.
Clinical trials of Contrave show that you can lose up to 5% of your body weight on Contrave. For best results, you should also eat a low-calorie diet and incorporate more physical activity into your daily routine while taking Contrave.
