Organ rejection begins with the body identifying a transplanted organ as “foreign,” and then attacks the organ as it would any other infection. Organ rejection happens when the immune system damages the organ enough to compromise its ability to function.
A critical component of immune reactions is the proliferation of white blood cells, the cells that actually attack infections or foreign tissues. When attacking an infection or other “invader,” white blood cells divide quickly so that a white blood cell army can attack the invader. The active ingredient in Imuran, azathioprine, blocks the ability of cells to copy their genetic information and divide into new cells. As a result, there will not be enough white blood cells to attack the newly-transplanted organ or, for rheumatoid arthritis, the joint tissue.
Imuran is first administered right after a transplant surgery and taken until discontinued—the best time to discontinue Imuran after transplant has not been determined. For rheumatoid arthritis, Imuran is given for up to 12 weeks. Patients should see improvement in four to six weeks.
Imuran suppresses the immune system, the first line of defense against infections. So Imuran lowers the body's ability to prevent or fight infections, putting patients at a higher risk of infections. Imuran also puts patients at risk for cancer, particularly lymphoma.
Other side effects include diarrhea, nausea, vomiting, stomach pain, hair loss, rash, loss of appetite, and mouth sores. Allergic reactions are rare, but you should watch for signs of an allergic reaction (rash, itching, wheezing) and immediately talk to your doctor.
