When the body breaks down ifosfamide and cyclophosphamide, one of the substances produced is acrolein. This chemical concentrates in the bladder and kills the cells in the tissue lining the inside of the bladder, producing sores and bleeding in the bladder.
When the body breaks down mesna, the active ingredient in Mesnex, one of the substances produced attaches to acrolein in the bladder, making it easier for the body to excrete it through urination. Ifosfamide breaks down into a second chemical that is toxic to the bladder. Mesna removes that chemical, as well.
Mesnex is administered as three injections or one injection and two tablets on the day that cyclophosphamide or ifosfamide is administered. The first is given when the chemotherapy drug is administered, the second a few hours later, and the third a few hours after that.
Most people will experience site reactions to the injection, such as pain, redness, or irritation. In general, the common side effects of Mesnex include, but are not limited to, loss of appetite, nausea, vomiting, diarrhea, constipation, swelling, drowsiness, stomach pain, chest pain, and pale skin.
Serious side effects include low blood pressure, anemia, and low potassium.
Allergic reactions are rare, but can be severe and include anaphylaxis, a dangerous drop in blood pressure, and severe skin reactions. Tell your physician if you notice fever, sore throat, rash, swelling of the face or mouth, racing heartbeats, mouth sores, or trouble breathing.
