What is Neostigmine Methylsulfate?

Neostigmine Methylsulfate is a generic prescription drug used to control symptoms of myasthenia gravis (a muscle weakness condition), prevent swelling and urinary retention after surgery, and reverse the effects of muscle relaxants used in surgery.

All of these conditions are due to nerves losing the ability to stimulate muscle contractions. Neostigmine Methylsulfate prevents an enzyme, called acetylcholinesterase, from breaking down a chemical, called acetylcholine, that is responsible for passing nerve signals to muscles to signal them to contract.

Myasthenia gravis is a rare condition in which the number of acetylcholine receptors in muscles declines. As a result, muscles receive “weaker” signals from the brain, resulting in overall muscle weakness. By preventing acetylcholine from being broken down, Neostigmine Methylsulfate allows the acetylcholine to linger at the remaining muscle receptors longer. More of the nerve signals are sent to the muscle, allowing greater movement and strength.

Muscle relaxants are given in surgery target these same receptors by increasing the activity of acetylcholinesterase, the enzyme that breaks down acetylcholine. Neostigmine Methylsulfate quickly reverses that effect, allowing patients to start moving muscles after surgery.

Neostigmine Methylsulfate is administered as an intravenous or intramuscular injection by a healthcare professional.

What are the side effects of Neostigmine Methylsulfate?

Many of the side effects are due to increased muscle sensitivity to nerve signals, especially in nerves that control involuntary muscles, like sweat glands. These side effects are very common and include increased sweating, increased salivation, increased mucus, decreased pupil size, increased urination, stomach and intestinal cramps, muscle cramps, and muscle twitching.

Other common side effects include nausea, vomiting, fainting, and drowsiness.

The most serious side effects of Neostigmine Methylsulfate are very rare but include slow heartbeat, cardiovascular complications, and convulsions.

Allergic reactions are possible and could be severe, including anaphylaxis, a dangerous drop in blood pressure.
