What is Plavix: Uses, Warnings & Interactions

Plavix is a brand-name prescription drug that prevents heart attack and stroke due to blood clots. Clopidogrel, its active ingredient, blocks the ability of blood platelets to clump together to start the blood clotting process. Daily doses are identical no matter what condition is being treated, but some people may have an initial dose that’s very large. Oral blood thinners are potentially hazardous drugs because of the risk of bleeding and blood loss. However, they keep people from having worse problems, so Plavix’s benefits outweigh the risks.

Healthcare professionals use Plavix to prevent heart attack or stroke in people with heart problems (acute coronary syndrome), poor circulation in their legs (peripheral arterial disease), or who have had a recent heart attack or stroke.

  • To prevent heart attack or stroke in people with acute coronary syndrome: 300 mg initial dose followed by 75 mg tablet once per day

How to take Plavix

  • Your doctor will tell you how much medicine to use. Do not use more than directed.

  • Please read the Medication Guide that comes with this drug. It contains useful information about taking the drug, its risks, and what signs to watch for.

  • Do not change the dose.

  • Do not miss doses or stop taking Plavix except under the advice of a doctor.

  • Take only one tablet per day.

  • Store Plavix at room temperature in a closed container.

What to do if you miss a dose of Plavix

  • If a dose of Plavix is missed, take it when remembered. If it’s almost time for the next dose, skip the missed dose and take the next dose as scheduled.

  • Do not take two doses to make up for a missed dose. 

Plavix contraindications

  • This medicine is not right for everyone. 

  • Do not use it if you have had an allergic reaction to clopidogrel or have any condition that causes bleeding, such as a stomach ulcer.

Plavix warnings

  • Because of the risk of bleeding, it is very important that any doctor, dentist, or other healthcare professional who treats you knows that you are taking Plavix.

  • Your doctor may need to perform regular check-ups and lab tests to monitor your progress and detect side effects or complications. Keep all appointments while taking Plavix.

  • Before starting clopidogrel, tell the prescriber if you are pregnant or breastfeeding. The prescriber should also know about all medical conditions you have or may have had, especially:

    • Any current bleeding problems like stomach ulcers

    • Any history of intestinal or stomach ulcers

    • Any history of bleeding problems or bleeding episodes

    • Any upcoming surgery or medical procedure, including dental procedures

    • Kidney disease

    • Heart failure

  • Plavix is hazardous because it makes you bleed and bruise more easily. Once bleeding starts, it will take longer than usual to stop. Immediately contact the prescribing doctor or get medical help if you experience:

    • Unexplained bleeding or bruising

    • Bleeding that lasts a long time

    • Blood in the urine

    • Blood in the stools

    • Black and tarry stools 

    • Coughing or vomiting up blood or blood clots (“coffee grounds” vomit)

  • To prevent unnecessary bleeding or bruising:

    • Take special care to avoid injury.

    • Avoid rough or risky activities.

    • Be gentle when brushing or flossing your teeth.

    • Avoid sharp objects. If you can’t avoid using a sharp object, be very careful.

  • This medicine may cause a rare but serious blood clotting condition called thrombotic thrombocytopenic purpura (TTP).

  • Do not stop taking Plavix until talking with the prescriber. Stopping the medicine too soon puts you at risk of heart attack or stroke.

  • Keep Plavix out of the reach of children.

Plavix interactions

  • To avoid potentially hazardous drug interactions, tell the prescribing healthcare provider about all prescription drugs, over-the-counter medications, and supplements being used, particularly:

    • Other blood thinners, such as warfarin

    • Nonsteroidal anti-inflammatory drugs (NSAIDs) such as aspirin or ibuprofen

    • Opioids

    • The stomach acid reducers omeprazole or esomeprazole

    • Antidepressants and anxiety medications called SSRIs or SNRIs

    • Repaglinide, rifampin, ritonavir

  • Tell other healthcare providers you are taking Plavix before they prescribe new medications.

Plavix side effects

Serious side effects

Talk to a doctor if you notice any signs or symptoms of a possible serious side effect, including:

  • Unexpected bleeding or bruising, bleeding that doesn’t stop

  • Bloody or black, tarry stools

  • Red, bloody, or dark brown urine

  • Coughing or vomiting up blood or blood clots

  • Severe abdominal pain

  • Pinpoint red or purple spots on your skin or in your mouth, pale skin, yellow skin or eyes, tiredness, fever, trouble breathing, fast heartbeat, headache

  • Allergic reaction: Itching or hives, swelling in your face or hands, swelling or tingling in your mouth or throat, chest tightness, trouble breathing

Less serious side effects

  • Nosebleeds

  • Bruises

  • Headache

  • Dizziness

  • Rash

  • Abdominal pain

  • Purple spots on the skin

  • Upset stomach
