What is Xcopri: Uses, Warnings & Interactions

Xcopri is a brand-name prescription anti-seizure medication. Its active ingredient is cenobamate, a drug that prevents seizures by slowing down nerves and reducing their excitability. Xcopri tablets are taken once daily as a maintenance treatment for partial-onset seizures but are not used to treat active seizures. They can be swallowed whole or crushed and mixed with water to make dosing easier.

How to take Xcopri

  • Follow all the prescriber’s instructions.

  • Please read the Medication Guide included with the medicine.

  • Patients start with three Xcopri titration packs, one for the first month of treatment, one for the second, and one for the third. 

    • Use the 12.5-25 pack for the first 28 days of treatment.

    • Start the 50-100 pack the day after finishing the first pack.

    • Start the 150-200 pack the day after finishing the second pack.

    • The tablets in each pack are numbered from one to 28.

    • Start with the tablet marked “1.”

    • Take all the tablets in order.

  • Take tablets once daily at any time of day.

  • Tablets can be taken with or without food.

  • Swallow the tablet whole with a drink of water.

  • If swallowing tablets is difficult, crush the tablet, mix it with water, and take it that way.

  • The tablet can also be crushed and mixed with water for use in a nasogastric feeding tube.

  • Do not stop taking doses until you’ve talked to the prescribing healthcare provider.

What to do if you miss a dose of Xcopri

  • Take a dose as soon as you remember. 

  • If it is almost time for your next dose, wait until then and take a regular dose. 

  • Do not take extra medicine to make up for a missed dose.

Xcopri contraindications

  • This medicine is not right for everyone. 

  • Do not use it if you have:

    • A heart rhythm disorder called familial short QT syndrome

    • Known hypersensitivity to cenobamate.

Xcopri warnings

  • Tell other healthcare professionals providing treatment that you are taking Xcopri.

  • The prescriber requires regular checkups and lab tests to monitor treatment and watch for side effects. Keep all appointments.

  • Before starting this medication, tell the prescriber if you are pregnant, breastfeeding, or have:

    • A history of depression, suicide, suicidal thoughts, or mood problems

    • Liver problems

    • Kidney problems

    • Blood problems

    • Any past allergic reaction that involved a skin rash or affected internal organs

  • If you become pregnant while taking Xcopri, immediately tell the prescriber.

  • Cenobamate can cause dizziness, drowsiness, poor coordination, and vision problems. 

    • Avoid driving, using machines, or doing anything else that could be dangerous until you know how the medicine affects you.

  • Do not stop using this medicine suddenly. The doctor will need to slowly decrease your dose for two weeks before you stop it completely.

  • Xcopri can be habit-forming and lead to abuse. 

    • Do not use more than prescribed. 

    • Call the prescriber if the drug does not seem to be working.

  • Xcopri is a federally controlled substance. Giving or selling this medicine to another person is dangerous and illegal.

Xcopri interactions

  • Before starting Xcopri, tell the prescribing healthcare provider about all prescription drugs, over-the-counter medicines, and supplements being taken, particularly:

    • Birth control pills

    • Other antiseizure drugs, including benzodiazepines or barbiturates

    • Drugs that make you sleepy, including alcohol, sleeping pills, narcotic pain relievers, and allergy medications

  • Do not drink alcohol while you are using this medicine.

  • Tell other prescribers about Xcopri before they prescribe new medications.

Xcopri side effects

Serious side effects

Talk to a doctor if you notice any signs or symptoms of a possible serious side effect, including:

  • Thoughts of suicide, suicide attempts, new or worse depression, new or worse anxiety, unusual changes in mood or behavior, unusual thoughts, trouble sleeping, aggressiveness, recklessness

  • Frequent or long-lasting infections

  • Severe fatigue, weakness

  • Severe muscle pain

  • Unusual bleeding or bruising

  • Dark urine or pale stools, nausea, vomiting, loss of appetite, stomach pain, yellow skin or eyes

  • Dizziness, trouble walking, trouble concentrating, vision problems

  • Fever, skin rash, facial swelling, swollen, painful, or tender lymph glands in the neck, armpit, or groin

  • Allergic reaction: Itching, hives, skin rash, swelling of your face, eyes, lips, or tongue, trouble breathing, trouble swallowing

Less serious side effects

  • Sleepiness

  • Dizziness

  • Changes in balance, clumsiness, or unsteadiness

  • Double vision

  • Headache
