Neffy is a prescription nasal spray that contains the active ingredient epinephrine. It is the first and only needle-free epinephrine treatment approved by the FDA for severe allergic reactions, which may be especially helpful for those with a fear of needle-based injections. Neffy, made by the company ARS Pharmaceuticals, is classified as an alpha and beta-adrenergic receptor agonist. It is FDA-approved to treat type I allergic reactions, including the treatment of anaphylaxis, in adults and children weighing 66 pounds or more.
Type I allergic reactions, also called immediate hypersensitivity reactions, happen quickly, usually within minutes after coming into contact with an allergen. This reaction is caused by the release of chemicals like histamine from cells in the body, leading to symptoms such as itching, swelling, and hives. Treatment typically includes antihistamines, corticosteroids, and, in severe cases, epinephrine.
Each Neffy has one dose of medicine for a one-time use. Do not test spray the device before using. Administer one spray of Neffy into one nostril, aiming straight up, and press the plunger firmly to activate. Do not sniff during or after using Neffy. If any liquid drips from your nose, you might not get the full dose. If symptoms worsen or do not improve, a second dose can be administered in the same nostril, but with a new nasal spray, 5 minutes later. Patients who are prescribed Neffy should always carry two Neffy nasal sprays with them.
Before giving you a prescription for Neffy, your healthcare provider will talk to you about when to get emergency help. If you have an allergic emergency, you should use Neffy right away. You should get emergency medical help for further treatment if needed after using Neffy. If you are not sure whether you need emergency medical help, it is best to err on the side of caution and call 911 for emergency medical assistance.
If you have certain medical conditions or take specific medicines, using Neffy might make your condition worse or cause more severe or longer-lasting side effects. Before using Neffy, tell your healthcare provider about all of your medical conditions, including if you are pregnant or breastfeeding. Be sure to mention if you have any problems with the nose, including nasal polyps or past nasal surgery.
Store Neffy at room temperature. Do not freeze, as freezing will prevent the device from working. Replace Neffy before the expiration date.
The most common side effects of Neffy are:
Throat irritation
Nasal tingling, itching, or discomfort
Runny nose
Stuffy nose (nasal congestion)
Headache
Excitement, nervousness, or anxiety
Tingling sensation
Fatigue or shaking
Sneezing
Abdominal pain, nausea, or vomiting
Dizziness
Gum pain or numbness in the mouth
Before using Neffy, tell your doctor about all the medicines you take, including prescription and OTC drugs, vitamins, and supplements. Be sure to mention if you are taking any of the following medicines.
Any nasal spray (Neffy may change the nasal mucosa for up to 2 weeks after use, which can increase how much other nasal products, including Neffy, are absorbed. This could raise the risk of side effects from these products.)
Diuretics (water pills)
Certain antidepressants (tricyclic antidepressants or MAOIs)
Medicines for heart disease or heart rhythm, including beta blockers such as propranolol
Medicines for Parkinson’s disease
Ergots for migraine
Thyroid medications, such as levothyroxine
Antihistamine allergy medications like Benadryl (diphenhydramine)
