What is Phospholine Iodide?

Phospholine Iodide is a brand-name prescription eye drops medication prescribed to patients with open-angle or closed-angle glaucoma and to diagnose or treat children with accommodative esotropia, or crossed-eyes due to the child's inability to focus on objects near the eye. Also called echotiophate, Phospholine Iodide belongs to a class of drugs called miotics that cause the iris to contract and narrow the pupils.

Both open-angle and closed-angle glaucoma is a rise in the pressure of the fluid inside the eye (intraocular fluid) due to blocked drainage canals at the outer edges of the iris in the eye. Instead of draining properly, the intraocular fluid builds up within the eye. Untreated, glaucoma will damage the optic nerve and result in vision loss. Phospholine Iodide causes the muscles in the iris to contract, narrowing the pupil. Contracting the iris helps open the drainage canals at the outer edge of the iris. As the fluid inside the eye drains, the pressure inside the eye decreases.

Accomodative esotropia is a condition in young and very young children in which the eyes cross because a far-sighted child tries to focus on near objects. As a result, the eyes are “trained” to cross and remain that way even when the child is not trying to focus on near objects. Esotropia over time will result in underdeveloped vision in one or both eyes.

Accomodative esotropia accounts for ½ of esotropia diagnoses in children and is most often successfully resolved with eyeglasses.

Phospholine Iodide is used to test for accommodative esotropia. The child is given Phospholine Iodide eye drops over a few days to see if this will resolve the crossed eyes. If it does, this means the crossed eyes are accommodative rather than due to some other cause.

The therapy of choice is to use eyeglasses to correct the inability to focus on near objects. If eyeglasses are not possible – for instance, in very young children – drugs like Phospholine Iodide that cause the iris to contract are the next line of therapy used before surgery.

The dose and treatment schedule will depend on the underlying condition.

What are the side effects of Phospholine Iodide?

The most common physical side effects of Phospholine Iodide are temporary site reactions on the surface of the eye including irritation, burning, blurred vision, watery eyes, and stinging. Other common side effects of Phospholine Iodide include but are not limited to eyelid muscle twitching, headache, puffy eyelids, and brow ache. Allergic reactions are very rare.
