A prescription bottle and pad: Topamax generic

Key takeaways

  • Topiramate is the generic version of brand-name Topamax, a medication for preventing seizures and migraines.

  • Generic topiramate usually costs much less than brand-name.

  • Ask your healthcare provider before switching this important neurologic drug.

Topamax (topiramate) is a brand-name prescription medication approved by the Food and Drug Administration (FDA) to treat certain types of seizures in adults and children ages 2 and older. It’s also approved to help prevent migraine in adults and adolescents ages 12 and older. A generic version, topiramate, has been available in the United States since 2009. This article explains how generic topiramate compares with brand-name Topamax in cost, effectiveness, and safety.

Is topiramate the same as Topamax?

“The active ingredient is the same,” Christina Le-Short, MD, medical director and vice president of clinical delivery at Override Health, says. “Generics are required to demonstrate bioequivalence, meaning similar absorption and overall exposure.”

Generic and brand-name drugs are expected to have the same active ingredient, strength, and route of administration. As an FDA-approved generic, topiramate works the same way as Topamax for seizure control and migraine prevention. “From a clinical perspective, most patients do just as well on generic topiramate,” Dr. Le-Short says.

The main differences are cost, appearance, dosage form options, and inactive ingredients, which can vary by manufacturer.

Dosage forms and strength

Brand-name Topamax is available as coated tablets and Topamax Sprinkle capsules. Generic topiramate is available as tablets, sprinkle capsules, extended-release capsules, and an oral solution. Other brand-name topiramate products include Trokendi XR, an extended-release capsule, and Eprontia, an oral solution.

The available dosage strengths among generic topiramate products are actually quite extensive, sometimes with even more options than the branded versions. Generic topiramate tablets and extended-release capsules, along with brand Topamax tablets and Trokendi XR capsules, come in 25 mg, 50 mg, 100 mg, and 200 mg strengths. Topamax Sprinkle capsules come in 15 mg and 25 mg strengths, but the generic option has 15 mg, 25 mg, and 50 mg options. Topiramate solution is only available in a 25 mg/mL concentrate regardless of whether it is the brand or generic product.

Appearance

Among the various topiramate versions, appearance is highly variable. Topamax tablets are round, coated, and color-coded by strength. Each tablet is stamped with “OMN.” Topamax Sprinkle capsules have “TOP” and the strength printed on them. Generic topiramate will look different depending on the manufacturer, formulation, and dosage.

Inactive ingredients

Topamax and generic topiramate may contain different inactive ingredients, such as binders, fillers, coatings, dyes, or flavorings. “Sometimes patients will think they are allergic to a medication when in fact it was an inactive ingredient that was the culprit,” Cooper Stone, DO, psychiatrist and clinical assistant professor of psychiatry and behavioral science at the Perelman School of Medicine at the University of Pennsylvania, says. If you have allergies or sensitivities to certain ingredients, ask your pharmacist or healthcare provider to review the label when switching products.

Efficacy

Generic topiramate is expected to work the same way as Topamax because FDA-approved generics must demonstrate bioequivalence to the brand-name drug. A small bioequivalence study comparing generic topiramate with Topamax found similar absorption between the products, and a small efficacy study found similar effectiveness.

Some research has raised concerns about patients with epilepsy switching between multiple generic manufacturers or from a brand to generic product, but these findings are limited and do not necessarily mean generic topiramate is ineffective. A broader review of seizure outcomes with generic versus brand-name antiepileptic drugs found no clear evidence that brand-name drugs were better at maintaining seizure control.

If you notice changes in seizure frequency, migraine control, or side effects after switching from Topamax to generic topiramate or from one generic to another, tell your healthcare provider.

How much does generic Topamax cost?

Generic topiramate is significantly more affordable than brand-name Topamax. Without insurance, brand-name Topamax costs about $520 for 60, 25 mg tablets. Generic topiramate prices vary by pharmacy, but SingleCare lists prices for the same strength and quantity starting as low as $8 with a coupon.

Using a SingleCare coupon can help lower prescription costs if you’re paying without insurance. SingleCare coupons are free and accepted at many retail pharmacies, including Kroger, Walmart, Walgreens, CVS, and Publix. Prices can change by pharmacy and location, so check the coupon price before filling your prescription.

Are Topamax and topiramate covered by insurance?

Most insurance plans, including Medicaid and Medicare Part D, are more likely to cover generic topiramate than brand-name Topamax. If your insurance doesn’t cover either medication or if your copay is high, using a SingleCare coupon instead may help lower the cost of topiramate. You can also ask your pharmacist or healthcare provider whether a manufacturer savings program applies to you.

If those options don’t work, talk to your healthcare provider about Topamax alternatives. Depending on whether you take Topamax for seizures or migraine prevention, alternatives may include other antiepileptic drugs, such as Keppra, Depakote, or Tegretol.

How to switch from Topamax to topiramate

Switching from Topamax to generic topiramate is usually straightforward, but it’s still worth discussing with your healthcare provider or pharmacist, especially if your seizures or migraine symptoms are well controlled.

“For patients with seizure disorders or very tight migraine control, consistency can absolutely matter, so it is reasonable to stay with one manufacturer if possible,” Le-Short, MD, says. “If a switch is made, it’s important to monitor for changes in side effects and efficacy over the next few weeks.”

Your pharmacist may be able to substitute generic topiramate automatically unless your prescriber says otherwise. To reduce the chance of switching between generic manufacturers, try to fill your prescription at the same pharmacy each time.

Important: Take topiramate exactly as prescribed

Take topiramate as prescribed, even if you feel well. Don’t stop taking it suddenly unless your healthcare provider tells you to, as this may increase the risk of seizures. After switching from Topamax to generic topiramate, contact your provider if you notice changes in seizure control, migraine frequency, mood, vision, sweating, body temperature, or cognitive function.

Expert takeaway

“In primary care, generic topiramate is often prescribed for migraine prevention and is chosen more often than brand Topamax due to better insurance coverage,” explains Chad Shaffer, MD, member of the SingleCare Medical Board. “Instead of brand Topamax, alternative medications are often chosen before generic topiramate and are substituted for it if ineffectiveness or side effects are a problem.”
