What is Prempro: Uses, Warnings & Interactions

Prempro is a brand-name hormonal treatment for women. It contains conjugated estrogens and a progestogen, relieves the symptoms of menopause, and helps prevent osteoporosis in postmenopausal women. However, Prempro can cause serious side effects or complications. Women should review these possible side effects with the prescriber and be familiar with warning signs to watch out for.

What is Prempro used for?

Prempro relieves menopausal vasomotor symptoms such as hot flashes and night sweats and treats menopausal changes in or around the vagina. Healthcare professionals also use it to prevent bone weakening (osteoporosis) in postmenopausal women.

What form(s) does Prempro come in

  • Tablets 

    • 0.3 mg conjugated estrogens/1.5 mg medroxyprogesterone

    • 0.45 mg/1.5 mg

    • 0.625 mg/2.5 mg

What are common Prempro doses?

  • For moderate to severe vasomotor symptoms of menopause: One tablet at the lowest effective dose taken daily

  • For moderate to severe vulvovaginal atrophy due to menopause: One tablet at the lowest effective dose taken daily

  • For prevention of postmenopausal osteoporosis: One tablet at the lowest effective dose taken daily

How to take Prempro

  • Your doctor will tell you how much medicine to use. Do not use more than directed.

  • Please read the Patient Information sheet that comes with the medicine.

  • Take only one tablet daily. 

  • Take the tablet at the same time every day.

  • Do not remove tablets from the blister pack until you’re about to take the tablet.

  • Swallow the tablet with a drink of water.

  • Store the tablets in their blister pack and original carton at room temperature.

What to do if you miss a dose of Prempro

  • Take a missed dose as soon as it’s remembered.

  • Skip the missed dose if it’s almost time for the next dose.

  • Do not take two tablets to make up for a missed dose.

Prempro contraindications

  • This medicine is not right for everyone. 

  • Do not use it if you have had an allergic reaction to estrogen or progestin, are pregnant, or have:

    • Abnormal vaginal bleeding that has not been checked by a doctor

    • Certain types of hormone-dependent cancers, such as breast or ovary cancer

    • A history of stroke

    • A history of heart attack

    • A history of liver disease

    • A bleeding disorder

Prempro warnings

  • Tell any doctor or other healthcare provider who treats you that you are using this medicine. You may need to stop using this medicine several days before you have surgery or need certain types of medical tests.

  • The doctor will need to regularly review the treatment with you, adjust the dose, and perform blood tests. Keep all appointments. 

  • Tell the doctor about all your medical conditions, particularly:

    • Unusual vaginal bleeding

    • Asthma

    • Epilepsy (seizures)

    • Diabetes

    • Migraine

    • Heart problems

    • Liver problems

    • Kidney problems

    • Thyroid problems

    • Parathyroid problems

    • High calcium

    • High blood pressure

    • High cholesterol or a family history of high cholesterol

    • Lupus 

    • Porphyria

    • Endometriosis

  • Estrogen therapy can cause serious problems, including:

    • Heart attack

    • Stroke

    • Blood clots

    • Breast cancer

    • Uterine cancer

    • Ovarian cancer

    • Enlargement of benign tumors of the uterus (fibroids)

    • Liver problems

    • Gallbladder problems

    • High blood pressure

    • High triglyceride levels 

    • Fluid retention

    • Depression

    • Severe allergic reactions

  • Immediately inform the prescriber if you have unusual vaginal bleeding.

  • Prempro can cause blood clots in the eye. Immediately call the prescriber if you have a sudden loss of vision, vision changes, or a bad headache. An eye doctor may need to check your eyes.

Prempro interactions

  • To avoid possible drug interactions, tell the prescribing healthcare provider about all the prescription drugs, over-the-counter medications, and supplements being taken, particularly:

  • Do not drink grapefruit juice when being treated with Prempro.

  • Tell other healthcare providers you are using this medicine before they prescribe new medications.

Serious Side Effects

Immediately contact a doctor or get medical attention if you notice any signs or symptoms of a possible serious side effect, including:

  • Unusual vaginal bleeding

  • Sudden or severe headache, numbness or weakness on one side of your body, problems with vision, speech, or walking

  • Severe pains in the chest or legs, weakness, fatigue

  • New breast lumps, breast discharge

  • Swelling in your face, hands, ankles, or feet

  • Seizures

  • Yellow skin or eyes

  • Allergic reaction: Hives, itching, swollen face, lips, or tongue, trouble breathing, abdominal pain, vomiting

Less Serious Side Effects

  • Headache

  • Breast pain or tenderness

  • Nausea

  • Vomiting

  • Irregular vaginal bleeding or spotting

  • Abdominal pain

  • Back pain

  • Gas

  • Itching
