Does doxycycline treat ear infections?
Doxycycline can treat some bacterial ear infections in adults, but it is not usually a first-line antibiotic for this condition. It is a tetracycline antibiotic that may be used when the bacteria causing the infection are likely to respond to doxycycline, especially if first-line antibiotics are not appropriate.
“For adults, it’s not really in my wheelhouse because it’s got very poor coverage against some of the bacteria,” says Dr. Matt Hershcovitch, MD, double board-certified ENT and founder of SoCal Breathe Free. “Doxycycline has variable activity against Haemophilus influenzae and Moraxella catarrhalis and definitely is not the first choice.”
Doxycycline does not treat viral ear infections because antibiotics do not kill viruses. Most ear infections in adults are bacterial, but a healthcare provider still needs to diagnose the type of ear infection before prescribing treatment.
Even when an ear infection is bacterial, antibiotics are not always needed. According to the CDC, some middle ear infections can improve with watchful waiting, rest, fluids, and over-the-counter pain relievers. However, severe middle ear infections or symptoms that last longer than two to three days may require antibiotics. In those cases, healthcare providers usually prescribe a first-line antibiotic, such as amoxicillin, before considering doxycycline.
What antibiotic is best for an ear infection?
For most uncomplicated bacterial middle ear infections in adults and children over age 2, amoxicillin is the standard first-line antibiotic. Doxycycline may be considered when first-line antibiotics are not appropriate, such as when a patient cannot take penicillin-based drugs or when another antibiotic has not worked.
“The preferred antibiotic for middle ear infections, and somebody who’s not allergic to penicillin, is amoxicillin,” says Dr. Rajesh Kakani, MD, Chief of ENT at Mount Sinai South Nassau. “And the preferred antibiotic for somebody with an outer ear canal infection is usually from the fluoroquinolones.”
After amoxicillin, “the next option would be amoxicillin-clavulanic acid, which is Augmentin,” Dr. Hershcovitch says, “and then if there’s a non-severe penicillin allergy, I’d do cefdinir or cefuroxime, which are cephalosporins.”
Certain circumstances may call for other amoxicillin alternatives, including antibiotic resistance, medication allergies, recent antibiotic use, or lack of improvement with first-line treatment. Some bacteria, including Staphylococcus aureus and E. coli, can develop antibiotic resistance, which may lead a healthcare provider to choose a different antibiotic. When is doxycycline prescribed for an ear infection?
Doxycycline may be prescribed for some bacterial middle ear infections in adults, but it is usually reserved for select cases. “Think of the ear as three organs,” Dr. Kakani says. The outer ear is related to the skin, the middle ear to the nose, and the inner ear to the brain. “So, believe it or not, the diseases of one usually don’t directly travel to the other, even though they are neighbors.”
The three types of ear infections often require different treatments. Outer ear infections are commonly treated with topical antibiotic ear drops, while middle ear infections may require an oral antibiotic. Inner ear infections, such as labyrinthitis or vestibular neuritis, are often viral and typically do not require antibiotics unless a healthcare provider suspects a bacterial cause. According to Dr. Kakani, “middle ear infections are usually affected by the bacteria that cause sinus infections,” such as Streptococcus pneumoniae, Haemophilus influenzae, or Moraxella catarrhalis.
“Now, if somebody is allergic to penicillin, we have to choose a different antibiotic,” Dr. Kakani says. “That’s where doxycycline may have some role in a very small selected number of patients.” If you develop symptoms of an allergic reaction, such as hives, itching, rash, or swelling, after taking amoxicillin, contact your healthcare provider right away. You should also contact your provider if your symptoms do not improve after a few days of treatment.
Doxycycline usually isn’t an option for children under eight years of age, since tetracycline antibiotics can cause permanent tooth discoloration and effects on bone development in young children, according to the American Academy of Pediatrics.
Mild ear infections might not require antibiotics at all, and the same often goes for outer ear infections. Inner ear infections, such as labyrinthitis or vestibular neuritis, are different from middle ear infections. They are often viral, so antibiotics like doxycycline are not typically used unless a healthcare provider suspects a bacterial cause.
Doxycycline dosage for ear infections in adults
According to FDA prescribing information, the usual adult dose of doxycycline is 200 mg by mouth on the first day, given as 100 mg every 12 hours or 50 mg every 6 hours. After that, the usual maintenance dose is 100 mg per day. For more severe infections, like otitis media, the typical dose is 100 mg every 12 hours.
Doxycycline comes in different forms, including doxycycline hyclate and doxycycline monohydrate. Both contain the same active drug and work the same way against susceptible bacteria, but they are different salt forms. Doxycycline hyclate is more water-soluble, while doxycycline monohydrate may be easier on the stomach for some people because it is absorbed more slowly. Neither form is specifically preferred for ear infections; the choice usually depends on the prescriber’s recommendation, availability, tolerability, and cost.
According to Dr. Hershcovitch, a typical course of doxycycline treatment is 10 days. Continue taking the medication for as long as your healthcare provider directs, even if you start feeling better before the medication is finished.
How long does doxycycline take to clear an ear infection?
Many people start to feel better within 48–72 hours of starting doxycycline, but the infection may take the full course of antibiotics to clear.
But that doesn’t mean you should stop taking it right away. Symptoms can improve before the infection is fully cleared, and stopping too soon may allow bacteria to keep growing or come back harder to treat. That’s why it’s important to finish the full course as prescribed.
If your symptoms don’t improve after 2–3 days, contact your healthcare provider. They might want to re-examine you and/or change courses with your medication.
Why can’t you lie down after taking doxycycline?
Right before bed may seem like an easy time to take a daily medication, but you should stay upright for at least 30-60 minutes after taking doxycycline. Take each dose with a full 8-ounce glass of water to help the tablet or capsule move into your stomach rather than sit in your esophagus. If doxycycline gets stuck in the esophagus, it can cause pill esophagitis, which may lead to chest pain, heartburn, painful swallowing, or a sensation of something stuck in your throat. Seek medical attention if you have severe chest pain lasting more than a few minutes, cannot swallow, or feel like a pill is stuck.

You can take doxycycline with or without food, but if it causes an upset stomach, taking it with a meal can help. Avoid taking it close to antacids, iron supplements, or supplements containing calcium, aluminum, magnesium, zinc, or bismuth subsalicylate, since these can reduce how much doxycycline your body absorbs.
Common side effects of doxycycline include nausea, vomiting, and diarrhea, as well as skin rash, headaches, and dizziness. Doxycycline can increase your skin’s sensitivity to sunlight. So use sunscreen, wear protective clothing, and avoid tanning beds while taking it. Antibiotics can also disrupt the body’s normal bacteria, which may increase the risk of oral thrush or vaginal yeast infections, including vulvovaginal candidiasis.
People who are pregnant, planning to become pregnant, or breastfeeding should tell their healthcare provider before taking doxycycline. Older research has raised concerns that tetracycline antibiotics may affect fetal bone growth. More recent research has found that first-trimester exposure to tetracycline antibiotics was not linked to a higher risk of major congenital malformations. In general, healthcare providers avoid doxycycline during pregnancy unless the potential benefit outweighs the risk.
