Clozaril is a brand-name prescription drug with the active ingredient, clozapine. Generic alternatives are available from manufacturers such as Teva, Mylan, Aurobindo, and other generic manufacturers.
Clozaril is a brand-name prescription neuroleptic (tranquilizer) that is primarily used to control the psychotic symptoms of schizophrenia, such as hallucinations (false perceptions) and delusions (false beliefs). Because Clozaril has serious and life-threatening side effects, Clozaril is prescribed only to patients with severe schizophrenia who have not responded to other therapies. Clozaril is also sometimes prescribed to reduce the suicide risk of patients with schizoaffective disorder who have a history of suicide attempts.
Clozaril works by blocking chemicals in the brain responsible for sending signals between nerves. This slows and partly shuts down certain “circuits” in the brain. Many psychotic symptoms are due to hyperactivity in the brain, so slowing down the brain helps reduce these symptoms.
Because of its serious and sometimes life-threatening side effects, patients cannot fill a Clozaril prescription until they have been accepted in the Clozapine REMS (Risk Evaluation and Mitigation Strategy) program. The prescribing physician will help enroll the patient in the program and provide information about the program and its prescription procedures. Eligibility in the program will require monthly monitoring of white blood cell counts.
Once enrolled, the patient or patient's caregiver will consult by phone with a case manager and a pharmacist from the pharmacy certified by the manufacturers. Necessary medical tests will also be sent to the pharmacist by the prescribing physician. Only after these consultations will the prescription be sent by overnight courier to the patient.
Clozaril has severe and sometimes life-threatening side effects. Chief among these is that Clozaril severely lowers white blood cell counts, putting the patient at risk for infections. Clozaril can also cause seizures, heart disease, slow heartbeat, and heart attack,
Most patients will experience side effects when taking Clozaril. The most common side effects of Clozaril are sleepiness, dry mouth, trouble swallowing, muscle spasms, twitching, shaking, involuntary movements, muscle stiffness, trouble walking, dizziness, vertigo, headache, drowsiness, and blurred vision. More serious but less common side effects include liver problems, kidney problems, high blood sugar, low blood pressure, and severe muscle stiffness. Long-term continual use of Clozaril often leads to tardive dyskinesia, unusual twitching, and involuntary movements, that could be permanent.
Mental and mood changes include trouble thinking, loss of coordination, delirium (severe confusion), obsessive thinking, and strange dreams. Allergic reactions are rare, but discontinue Clozaril if there is any sign of an allergic reaction, such as hives, facial swelling, or difficulty breathing.
This is not a complete list of side effects that may occur. Consult with your physician about any concerns you may have with side effects.
