Not long ago, oral Wegovy (semaglutide) was the only needle-free GLP-1 receptor agonist medication for chronic weight management. In April 2026, a GLP-1 tablet called Foundayo (orforglipron) hit the market. Now that there are two GLP-1 pills for weight loss, patients have a real choice and a few key differences to weigh before talking to a healthcare provider. 

Key differences

The biggest difference between Foundayo and oral Wegovy is dosing convenience. Foundayo is effective even when taken at any time of day, with or without food, while the Wegovy pill requires a strict morning fasting protocol. Other differences include the manufacturer, the active ingredient, and average weight-loss results in clinical trials.

That said, Foundayo and Wegovy share more similarities than differences. Both are oral GLP-1 medications taken once daily, and both are approved for weight management in adults with obesity or who are overweight and have at least one weight-related health condition.

Conditions treated

Both Foundayo and oral Wegovy are approved by the Food and Drug Administration (FDA) for chronic weight management in adults. Oral Wegovy is also FDA-approved to reduce the risk of major cardiovascular complications (heart attack, stroke, or death) in adults with established heart disease who also have obesity or are overweight.

The injectable form of Wegovy is also approved to treat certain types of fatty liver disease, but the oral Wegovy pill is not. Neither Foundayo nor Wegovy is approved to treat Type 2 diabetes. Only semaglutide, sold under the brand name Ozempic, is approved for diabetes management.

How to take Foundayo

Foundayo is a once-daily tablet taken by mouth and swallowed whole — never crushed or chewed. Unlike semaglutide-based pills such as Wegovy, Foundayo has no timing or food restrictions. It can be taken at any time of day, with or without food, and with any amount of water.

“Foundayo having some flexibility is huge,” says Supriya Rao, MD, a board-certified gastroenterologist, obesity medicine, and lifestyle medicine physician in the greater Boston area. “Being able to take it without food and at any time of the day makes it easier to maintain. But, the best medication for the patient is the one they can realistically stick to.”

According to the prescribing information, Foundayo dosage starts at 0.8 milligrams (mg) once daily. The dose can be increased every 30 days until a maintenance dose of 5.5 mg, 9 mg, 14.5 mg, or 17.2 mg is reached. The maximum Foundayo dosage is 17.2 mg per day.

How to take Wegovy

The Wegovy pill is also taken once daily, but it has strict timing and fasting requirements. “Oral Wegovy won’t work as well if a patient eats or drinks too soon after,” Dr. Rao says. 

Wegovy must be taken first thing in the morning, on an empty stomach, with no more than 4 ounces (120 milliliters) of water. Patients must then wait 30 minutes before eating, drinking, or taking any other medication or supplement. This fasting window matters because oral semaglutide’s absorption is significantly reduced when taken with food or larger amounts of liquid. Dr. Rao says patients who travel frequently or take multiple medications might find these restrictions challenging.

According to the prescribing information, oral Wegovy dosage starts at 1.5 mg once daily. The dose can be increased every 30 days, escalating from 4 mg to 9 mg to 25 mg, until a maintenance dose is reached. The maximum Wegovy pill dose is 25 mg per day.

Is Wegovy better than Foundayo for weight loss?  

In separate clinical trials, the Wegovy pill led to slightly greater weight loss than Foundayo. At the highest approved doses, oral Wegovy reduced body weight by approximately 13.6%, while Foundayo reduced body weight by approximately 12.4%. 

It’s important to understand that these comparisons are indirect. There have been no head-to-head trials comparing Foundayo and Wegovy. That said, Novo Nordisk, the manufacturer of Wegovy, published an indirect comparison of weight-loss results from oforglipron versus oral semaglutide at the highest doses. The analysis suggested that Wegovy can drive approximately 3% more weight loss than Foundayo. 

Research is ongoing, and individual results may vary. “Sustainable, long-term progress is more important than chasing the highest percentage of weight loss,” Dr. Rao says. “It’s a case-by-case scenario.”

Foundayo vs. Wegovy: Insurance coverage and cost comparison

The out-of-pocket cost of Foundayo and Wegovy varies depending on dose, pharmacy price, and insurance coverage. Weight loss medications are not always covered by insurance; commercial plans are not required to cover medications that treat obesity, according to the National Institute of Diabetes and Digestive and Kidney Diseases. Foundayo vs. Wegovy side effects

Foundayo and Wegovy share similar side effects, most of which tend to go away as the body adjusts to the medication. Both medications are started at a small dosage and slowly titrated upward to help decrease adverse side effects.

CYP3A4 inhibitors (like clarithromycin, diltiazem, and verapamil) and inducers (like rifampin, carbamazepine, and phenytoin) affect Foundayo’s own levels, not the levels of other medications. The dose adjustments are for Foundayo itself, not to the CYP3A4 inhibitors or inducers. If you take Foundayo, the maximum dose of the cholesterol-lowering drug simvastatin should be 20 mg.

The drug labels for Foundayo and Wegovy warn against taking them if you have a personal or family history of medullary thyroid cancer or MEN2 syndrome. These medications should not be taken during pregnancy or breastfeeding. 

Which medication is right for you?

The decision between Foundayo and oral Wegovy should be made jointly by the patient and the healthcare provider. Several factors go into this decision, including the patient’s nutrition and exercise habits, schedule, budget, and other medications.

“I may choose Foundayo over Wegovy if patients desire an oral option but are opposed to the early morning dosing and delayed oral intake, if they desire flexibility due to an irregular schedule, and if their weight loss goals are less urgent,” says Chrissie Ott, MD, a board-certified internist, pediatrician, and obesity medicine physician in Portland, Oregon.

Dr. Rao says patients should think beyond pure efficacy numbers. “I don’t want patients to look at which medication causes the most weight loss the quickest and pick that one,” she says. “Patients need to think about which one would realistically fit their lifestyle and their health goals.”

Cost is another factor. Dr. Ott says either Foundayo or Wegovy can be a great choice for weight loss, but she would consider which is a better fit for her patient’s budget or insurance coverage. “I would also note that there are more steps to target dose on Foundayo, so that may be relevant in that it may take longer to see maximal effects,” she adds.
