Purple inhaler: How much does Breztri cost with Medicare?

Key takeaways

  • Most Medicare Part D and Medicare Advantage drug plans will cover Breztri, but it is often on the higher tiers of the formulary, which can make copays more expensive.

  • Prescription medications like Breztri are often expensive due to the complexity of manufacturing the brand-name drug, active patents, and insurance companies that either do not cover the drug or require beneficiaries to pay higher copays.

  • To save on Breztri, Medicare recipients can compare Medicare plans annually, research patient assistance programs, use a SingleCare discount card, or explore state-specific programs.

Bretztri Aerosphere (budesonide/glycopyrrolate/formoterol fumarate) is a brand-name, prescription-only drug approved by the U.S. Food and Drug Administration (FDA) for the treatment of a variety of chronic respiratory conditions. Healthcare providers may prescribe the inhaler medication to manage chronic obstructive pulmonary disease (COPD), chronic bronchitis, and emphysema. If you’re enrolled in a Medicare Part D plan, you can expect to pay around $47 for a monthly prescription on average.

Why is Breztri so expensive?

Without health insurance coverage, a Breztri inhaler can be pretty expensive, with the drug’s manufacturer, AstraZeneca, sharing that the list price for a 30-day supply is $665

Several factors influence the price of prescription drugs. In Breztri’s case, its high price point is mainly due to the complexity of producing the medication, explains Eleonora Fedonenko, MD, an internal medicine physician and medical director at Your Laser Skin Care in Los Angeles, California. “The drug combines three different medications into one single-use inhaler,” she explains. “Each time you take a breath, each puff has to be delivered with the same amount of medication to the lungs. This consistency requires a very high level of production as well as airtight packaging and 24/7 inspection to ensure that the product will remain safe.”

Another reason prescription medications like Breztri can be so expensive is the presence of active patents. The prescription inhaler has several active patents, with some expiring as soon as 2030 and some as late as 2038. “Breztri Aerosphere is expensive because there is no generic version of the drug,” says Ayesha Bryant, MD, MSPH, a physician with a background in epidemiology, cardiothoracic surgery, and perioperative medicine, and clinical advisor at Alpas Wellness in La Plata, Maryland. “This lets AstraZeneca, the manufacturer, control pricing and allows them to recoup costs that went into the research, development, and production of the drug.”

Lastly, insurance coverage can affect the costs of prescription drugs. “A typical patient who pays cash will pay approximately $600 per month, while a patient who uses insurance will typically spend on average of $200,” Dr. Fedonenko says.

Does Medicare cover Breztri?

Most Medicare Part D and Medicare Advantage (Part C) drug plans will typically cover the inhaler medication. In fact, Breztri reports that the drug is covered without prior authorization (PA) for 8 out of 10 Medicare Part D and commercial or private insurance recipients nationwide. However, coverage heavily depends on an insurance plan’s drug formulary, which is the list of medications covered by a health plan, often organized into cost tiers to guide beneficiaries toward the most cost-effective options. According to the Breztri website, most Medicare drug plans place the inhaler drug on tiers 3 and 4, or don’t cover it at all. Tier 3 typically covers preferred brand-name drugs and a few higher-cost generics, while Tier 4 comprises non-preferred drugs, including high-cost brand-name and generic medications

How much does Breztri cost with Medicare?

According to AstraZeneca, Medicare Part D recipients pay an average out-of-pocket cost of $47 per month. Eighty-one percent of people with Medicare Part D pay $50 or less out of pocket each month for their Breztri prescription. Medicare Advantage drug plans can have similar prices for the inhaler medication.

The actual cost you’ll pay for Breztri with Medicare coverage varies. “For those on Medicare, your out-of-pocket costs depend on your plan’s formulary, tier, deductible, and pharmacy network,” Dr. Bryant says. “Generally, higher formulary tiers and using any out-of-network pharmacies mean that the cost of the drug will increase.”

For specific costs, it’s best to contact your insurance provider for more details, as individual plans can have varying copays

Strategies to reduce Breztri costs on Medicare

If you have a Medicare Part D or Medicare Advantage plan, but your copays are too high, or your plan doesn’t cover Breztri, there are several options you can explore to help lower your out-of-pocket expenses for the inhaler medication.

Here are some methods you can use to cut back costs on Breztri while on Medicare:

Compare Medicare drug plans annually

During Medicare Open Enrollment, take the time to research eligible plans to see which ones will provide the best coverage with the fewest restrictions for your prescription drugs, including Breztri. “For more affordable options and cutting costs, I recommend that patients compare plans during periods of open enrollment and choose a plan that lists Breztri on a lower tier or offers savings through preferred pharmacies,” Dr. Bryant says.

Research patient assistance programs

Patient assistance programs can help you lower the costs of Breztri. AZ&Me Prescription Savings Program helps people who are uninsured or have Medicare access to AstraZeneca medications, including Breztri. Eligibility requirements include being a U.S. resident, receiving care from a licensed healthcare provider, not having commercial insurance, and not receiving any other medication payment assistance.

If eligible, you’ll receive the inhaler medication at no cost and will remain enrolled in the program for an entire year, with the option to reapply. Your prescription medications will be delivered to your home or your healthcare provider’s office.

You can also find patient assistance programs through the following resources:

Use a SingleCare discount card

Using Breztri Aerosphere coupons can also help you lower costs if your Medicare plan doesn’t cover your prescription. With the SingleCare discount card, you can pay $573 at Kroger for 1, 10.7 gm of 160-9-4.8 mcg/act (28 inhalations) of Breztri.

If you do have a Medicare Part D or Medicare Advantage plan that covers Breztri, but your copay is too expensive, using a SingleCare savings card might give you a lower copay for the inhalation aerosol medication than your insurance plan

Please note that you cannot use a discount card in conjunction with your Medicare drug coverage. Additionally, any money spent out of pocket when using a discount card at your pharmacy will not count toward your Medicare Part D deductible or annual out-of-pocket maximum.

To get this Breztri coupon for Medicare patients, visit the drug’s coupon page, pick your preferred pharmacy, and print or download the coupon to your mobile device. Present the coupon to your pharmacist to get your Breztri discount. The SingleCare prescription discount card is accepted at most major pharmacies nationwide.

Medicare Extra Help

If you’re enrolled in Medicare Part D and have limited income and assets, you might be eligible for Extra Help. This federally-administered program helps beneficiaries pay for copays, coinsurance, premiums, and deductibles.

You may already be enrolled in the Extra Help program if:

  • You have full Medicaid coverage; or
  • You receive assistance from your state to pay for your Part B premiums; or
  • You receive Supplemental Security Income (SSI) payments from Social Security.

If you’re not automatically enrolled in the Extra Help program, you can apply for the program online. If accepted, you will likely pay a lower copay or coinsurance for Breztri.

Explore state-specific programs

State Pharmaceutical Assistance Programs (SPAPs) are state-run patient assistance programs that help lower-income adults and individuals with disabilities afford their prescription medications. Some SPAPs may offer wraparound prescription drug coverage for Medicare recipients whose plans may not cover drugs like Breztri. To apply for your state’s SPAP, first check whether it provides one, as not all states have one. Next, make sure you meet the eligibility requirements (e.g., household income, age, health conditions, and insurance coverage), then apply.

If you have any questions about the costs of your Breztri medication, please contact your health insurance plan. For concerns regarding the inhaler medication, please consult your healthcare provider for professional medical advice.
